TLC Benefit Solutions, Inc. partnered with Chancy Drugs for the new Wellness Diabetes Management Program. The program is for employees and family members who have been diagnosed with diabetes and associated chronic disease states. Participants will meet with a Health Advocate and/or pharmacist to help manage their chronic conditions. All information exchanged with your Health Advocate and/or pharmacist is confidential and privacy is a priority.
Please note that if you are a diabetic and you choose not to participate in this program, you will be assessed a surcharge of $40.00 per month to your health insurance premium.
Benefits of the program include:
$5.00 co-pay for all diabetic medications, testing supplies, and medications used to treat associated chronic diseases such as high blood pressure, high cholesterol, sleep apnea, heart disease and asthma;
Glucose monitors will be provided free of charge to participants;
Chancy Drugs will provide a comprehensive medical review of your medications;
Chancy Drugs will also enroll you in their Ready Med synchronization program at no charge.
Requirements of the program include:
Medications must be filled at Chancy Drugs located in Hahira, Adel, Valdosta or Lake Park, and Moultrie (Medications will be mailed if you do not reside in near proximity to these locations)
Attend one educational class per quarter covering nutrition, general diabetes, exercise, blood sugar readings and impaired healing (Contact Chancy Drugs for class schedule)
Take A1C as required
Monitor blood sugar before eating breakfast and two hours after supper and provide readings to Chancy Drugs. Patients with Type 1 diabetes may require readings up to 8 times daily. Physicians may have their patients that are having difficulty controlling their blood sugar test more frequently or at different times of the day.
If applicable to condition, utilize remote monitoring equipment and tablet to take blood sugar readings, which will replace your written blood sugar log. You may also be provided with additional devises based on your medical history, such as a blood pressure monitor and a weight scale to submit these readings electronically to the health advocate.
All remote monitoring equipment must be returned to the Company if you choose to leave the program.
